- Business information — your industry, type of work performed, number of employees, years in business, legal entity type, Federal Employer Identification Number (FEIN/EIN), estimated payroll, ZIP code, states of operation, and requested coverage effective date;
- Insurance history — whether you currently carry workers’ compensation coverage, when you need coverage, and any prior claims or loss history you provide, including through supplemental forms such as the Loss History Affidavit, Staffing Supplemental, and Client List.
You provide this information voluntarily. Please do not submit sensitive personal information beyond what a form requests.
